Autism Care Navigator

Job Introduction

Make a real difference to autistic people across Bath & North East Somerset, Swindon and Wiltshire.
In this role, you’ll provide needs‑led support and tailored interventions to individuals accessing the BSW Adult Autism Service, part of the Neurodevelopmental Assessment Pathway.

Based in Bath, you’ll work flexibly - visiting locations across B&NES, Swindon and Wiltshire as needed, while also benefiting from opportunities for remote working.

Key Responsibilities

  • Conduct welcome calls with individuals referred for diagnostic assessment, gathering brief background information and identifying any required reasonable adjustments.
  • Gather information from service users, families, and significant others to support the multidisciplinary team’s clinical decision‑making.
  • Ensure service users and families can access recommended advice, resources, and support, offering additional help to those who have not yet engaged.
  • Respond to service user enquiries and work collaboratively with partner organisations, including VCFSE agencies, to ensure the right support is in place at the right time.
  • Promote a needs‑led, “think family,” trauma‑informed approach in all aspects of care.
  • Support service users and families to better understand autism and identify any needs that may affect their ability to access appointments within the pathway.
  • Assist the team with organising, monitoring, and delivering appropriate follow‑up arrangements.

Essential

  • Level 4 qualification in a relevant area (e.g., Higher Apprenticeship, Level 4 Award/NVQ/Diploma).
  • Strong interpersonal skills, including active listening, observation, and empathy.
  • Confident in negotiation, problem‑solving, and prioritising workload.
  • Excellent written and verbal communication skills, with the ability to record information accurately and sensitively.
  • Good organisational skills and the ability to work independently.
  • Experience in working with Autistic people.
  • Experience building effective relationships with staff from other agencies or organisations.
  • Confident using IT for tasks such as report writing and communication.
  • Experience supporting individuals with autism and/or ADHD

Desirable

  • Experience working in mainstream or specialist education settings.
  • Experience delivering training.
  • Experience contributing to neurodevelopmental diagnostic assessments for children, young people, or adults.

Other Requirements

  • A valid UK driving licence is required to fulfil the duties of the role.
