Job Introduction
As an Advanced Clinical Practitioner on the Wards in Sittingbourne Community Hospital you’ll join a team of support workers/HCAs, physiotherapists, occupational therapists and nurses who are focused on exceeding the needs of patients within our North Kent community.
This role covers both services in Swale, you may need to work in both Sittingbourne Community Hospital and Sheppey Community Hospital.
You will support the delivery of care to a range of complex patients which will include: life limiting conditions like diabetes, in dwelling long term catheter, urgent wound care etc.
This is a weekday service, working Monday - Friday, 9am-5pm. No weekends or bank holidays required.
Main Responsibility
As the Advanced Clinical Practitioner you will be working closely with a multidisciplinary team where you will use your experience to practice autonomously and flexibly. Practitioners should have significant experience in primary care or A&E/Minor Injuries Unit. Working within the nursing team, you will work with the service manager, and clinical lead to deliver robust clinical care.
The Ideal Candidate
You will have an Advanced Clinical Practitioner qualification, be NMC registered and hold a prescribing qualification.
You will also need to be confident using IT systems and be able to demonstrate working knowledge of Microsoft Office packages i.e. Outlook, Word, Excel.
